95 4.0L V6 Dies after idle and occasionally at low speed/rpm

The main problem is that the engine immediately cuts after starting, to counteract I give it gas up to around 2000rpm for a minute or so then it is good to drive. I drive it for ~10 mins to work and it'll die when I take my foot off the throttle and/or do a low-speed turn. I can "fix it" by cranking my throttle up to ~2000 rpms at idle (throttle cable housing has a break so I can pull it over the lip and keep it high) but this is a VERY short term fix.

I am running a P0171 code and banks 1 & 2 are lean. I recently replaced the intake manifold gasket vacuum leak + IACV valve and to my knowledge don't have other vacuum leaks, though my vac reservoir to the cabin is going bad. On the highway the truck performs just fine, I drove 1300 miles to Texas and back this weekend. Fuel filter < 5000 miles old. Do have a pretty bad power steering fluid leak too. After the truck dies and I crank it back from neutral it will have a back and forth rocking and the idle will fluctuate below 500rpms. I'm thinking maybe the fuel injectors or pump are going bad, or it's an O2 sensor.

Anyone had a similar problem or know what it might be? Thanks.
 
Take a propane torch and hook a length of rubber fuel line and turn it on low without lighting it. Run the hose all around the vacuum hoses and around the throttle base and manifold. If you have a leak the engine will speed up. Stay away from the plug wires, might have a leak and ignite the propane. It sounds like a vacuum leak.
