Sudden Rattling After Oil Change

So I changed my oil the other day and the Oil Light started flickering from time to time. That has stopped. But now I have a rattle! It rattles when fist started after sitting for a while. It also rattles as the RPM's peak right before a shift. Not all the time though! Is it possible I just have a bad tensioner that is bleeding down and not fully charging? I did replace them at about 260k and it has 268k now. Should I replace them again? I just hope it's not the "Rattle of Death"! My 2003 V8 Ranger is almost done and I need this one to last a couple more months.

Is the oil full? What weight oil did you use? What kind of filter?

Rattle usually means guide is already broken

Yes, change right rear tensioner first

And do this on startup from now until you change it
"Clear Flooded Engine" mode
Key on
Press gas pedal to the floor and hold it down all the way
Crank engine over to get oil pressure/circulation, engine should NOT start, fuel injectors are OFF
Crank 5 to 10 seconds
Release gas pedal, while cranking or after cranking and engine will start with some oil pressure already built up

Is the oil full? What weight oil did you use? What kind of filter?
Yes, oil is full. I use Mobil1 5w-30 and a Mobil1 filter.

The rattle comes and goes now! I don't think the guide is broken but I've ordered both tensioners. I'll try the starting method until I can get them changed.
