There is a stud that is welded on the bottom of the mount. You put a flat washer and a nut on the bottom side, I used grade 8. That is all there is to it.I don't believe with proper hardware this will happen. It certainly hasn't happened with mine.
Hardware isn't an issue.
I am going to guess you lucked out when cut your slots.
