Intermittent vibrations at highway speeds

Ive had the wheels balanced. The entire front end suspension had been redone and the tires are new so uneven tread is not an issue.. when putting on the new tires the guy at the shop said one of my wheels is bent and i will get still some vibrations. If thats the case wouldn't the vibrations be continuous? Sometimes i feel it only in the cabin, others only in the steering wheel and sometimes i dont feel it at all..could a bad axel/rear end also be contributing here?
 
Find the bent wheel and take it off and put your spare on and run it and see what it does.

P.S. That wheel was bent somehow. Was it bent on this truck? If so, what else got damaged? Ever get it aligned?
