Radius arm bushings can make horrid noises when they're shot, I know, I've been driving around with fried ones on my '90 for YEARS :). If that was the case the noise would be below the drivers or passenger foot area

There are several more candidates including a loose u-joint or a loose driveshaft slip joint that can make clunk type noises when load goes on/off.

I’m buying a bunch of parts tonight I do need a carrier bearing but all my U joints are tight should I also just throw a set of radius bushings on as well ?

Okay well I’ll just get a carrier bearing and hope that fixes it I checked my leaf springs and they are all good so if it’s something else then I have no clue

Also this truck has a tick that it only dose for a couple seconds then stops for a couple seconds and you can barely hear it from the engine bay so it’s not lifters or timing you only hear it in the cab or under the truck by the bell housing any ideas what that could be? It also dose speed up as the engine revs up

Ok, you obviously have a lift, did they happen to stack blocks in the rear suspension? If so axle wrap is going to be a serious issue and can cause the U joint caps to contact the flange or something dumb, heck my '90 the driveline hits the gas tank skidplate sometimes...

The early 4.0L's were known to wear out pushrod ends and rocker arms

Yeah it dose have a old wire out 4in rough country with blocks in the rear but they don’t look like the ones rough country sells now but it’s also the first variant they came out with so idk and yeah it thought about push rods but if that was the case why would I only hear noise in the cab and by the bell housing

cheap test, try manually rocking it.
park on hard smooth pavement, leave it in 1st gear without the parking brake set.
grab the top of a front tire (gives you leverage) and start rocking it.
get a rhythm going, start the opposite pull just as it bounces off the stop.
maybe also try 4th gear because it's straight thru thus eliminates some tranny parts. the engine may turn a bit in 4th.
don't tell the neighbors what you're doing, keep 'em guessing about your sanity.
