Not to long ago I replaced the cam shaft synchronizer because mine was squeeling. I got the part at NAPA for about $70. A buddy of mine who is a mechanic put it in. He told me there were basically only two ways it could go in, correct or 180 off. Is that true? He also said that if it was 180 off the engine would still run but it would run rough and probably would kick the check engine light. Again, is that correct and if so, how rough are we talkin? Major, barely noticeable, only at idle, etc etc???

It can be installed in as many positions as there are teeth on the Synchronizer drive gear. An incorrectly timed CMP sensor may cause an off idle hesitation or not, depending on the error. It can also cause a CEL.

In other words, it could idle a bit rough? What about mileage?

If you're worried about it, why not check it? The Chinese Synchronizer kits usually come with a timing tool in the package. Presumably, that's what NAPA is selling.
