Alright, to start off I've been having issues for a month now. The truck will start up well every time, but usually after several minutes of driving there will be a loss of power, seeming and literally sounding like someone threw a choke on. If I pop it into neutral the engine will stall and sputter until it dies. I pulled codes and got a code 13 (idle out of range) and code 41 (running lean).
Checked for vacuum leaks and couldn't find any, so I continued to look for exhaust leaks and ended up seeing some carbon buildup on the connection between the driver side header and the exhaust pipe.
Im posting to either confirm my suspicion, it get other ideas from people with similar problems.

How long since it has all been apart last? The easiest fix is to take it out and put orange silicone around the cups.

Well, I only got the truck in March, I haven't messed with it much. No telling when the last owner did, my only knowledge being he beat the hell out of this thing.

That would be the high temp. copper silicon right?
 
Yes, high temp copper.
 
A little update. After taking a better look at it, I'm starting to think this might have something to do with it. Lol.

Looks like that bolt is pretty damn loose.

Yeah, that bolt does look pretty loose. Back it out carefully, add a washer or two and some anti-seize and get her in tight.
 
Well got it in there nice and tight, but I'm still having the same issues. I checked the connection between the manifold and exhaust on the other side, and what do you know, it's loose too. I'll do the same to the other side, but I'm starting to feel like it might not be the problem. I feel like maybe a clogged cat, or bad sensor.
