Cause for broken lock nut washer?

Yesterday I took a look at my bearing because they were getting loud. Come to find out that the washer between my two lock nuts broke. (lock nuts I am talking about are here). The nipple on the washer that keeps it from spinning on the spindle broke. The metal that broke off obviously went into the bearings.

What could cause the washer to break? I torqued the outer nut down has hard as I could. (about 150 foot pounds).

I don't think I've ever heard of the tab (nipple?) on a washer actually breaking...

Is it possible what happened was the tab came up out of the groove onto the spindle threads while you were tightening it?
 
Hah, Yes Tab. Me neither. When I put the hub on all the nuts and washers were looking good. I drove a few thousand and then it happened. I suppose I could blame it on the Michigan roads..

Justently had one that the pin was pushed to the wrong side. Well....maybe installed upside down.... The pin was centered so that it wasnt usable. The nut was torqued down while the pin wasn't in a hole. Anyway I pushed the pin back properly. The pins are a press fit and can be moved with a pin punch, so make sure that isn't what happened.

Just figured out what I did.. I did it again. Took an impact on the second nut spinning both nuts taking that center washer with it. doh. haha. Thanks for the help! :)
