Can you tell if this is body or sus lift?

No Problem.

Yup, definitely a suspension lift (drop extensions on the radius arm crossmember)
 
I remember putting those in!

Yes, BIIs broke apart from the Rangers slightly in design, there's a number of differences here and there....

BIIs used no overload leaf in the packs and they used no blocks, stock. They just had a soft like 5 leaf pack.

2wd BIIs had a 2" lift coil front, but the same spring rate as a 4x4 (or at least my '89 did).

2wd BIIs got a dummy fully machined BW 1350 T-case and 4x4 trans. They were also built on 4x4 frames.
