bed tie downs

I took my bed liner out & when doing this I broke off all the tie down cleat bolts this has happened to me before & I didn't fix them vary well so here I am again so could anyone tell me the best thing to fix them (01 ranger)

I have tie down inserts in the 4 "stake" holes in the bed, they are flush when closed, so bedliner and shell are no problem.
I also made 2x4 rails that are held in place by the above tie downs and the 2x4s have multiple attachment points as needed
