slight stumble

Let my step daughter use my truck for about 2 months after surgery, she had to drive a automatic. After I got it back, it had a stutter at 1st push of the gas pedal. Had codes that read motor was lean , traced it to a cracked pcv hose. Replaced that, reset codes. Stutter still there, no codes showing
replaced plugs, used Sea Foam, cleaned cone air filter, checked/cleaned IAC.
looked for vacuum leaks, none.( I did see an small hair line crack in the MAF )
Occasionally, while driving, the truck would have a hiccup, best way I can describe how it would feel, it's as if someone kicked it in the back end.
She did mention one day it was shard to start, as she said " I crunked it till it started" and she mentioned black smoke came out the tailpipe and the CEL came on.
truck has 184000 miles ,mostly highway miles
02 3liter flex fuel, automatic
plug wires replaced 2 years ago
Besides replacing the MAF, any ideas suggestions ?

If the syncronizer shaft is original, you may want to check it out. Do you get the "normal" belt whine whenthe engine is cold? Does it come back at stop after traveling hiway speeds in colder weather? Syncro is not monitored for faults over 2500rpm per Ford TSB.
