I'm getting ready to replace the lower ball joints on my '03 4x4. Will I need to disconnect the axle shaft to move the knuckle out of the way or will there be enough room if I keep it attached?

You can do it with just moving the knuckle out of the way. It’s well worth the extra work to just pull the knuckle out and give yourself all the room you need instead of fighting for space and the right angle.

I was thinking about replacing the ball joints in my 2002 4WD when I change out the steering knuckles for the larger 2004 front disc brakes.
My manual says that the ball joints come with the control arm - a much larger job to change. Is that really necessary? I see that Rockauto.com sells the ball joints by themselves though I don't see how they are fastened to the control arm - bolts, rivets, etc.
