My first diesel, 89 E350 cutaway with a 7.3 IDI and an E4OD. Love it, but it really lacked in power and I could not get it to downshift with throttle. Would not hold speed uphill. Did some searching and the suggestion was the FLP sensor, (fuel lever position). In checking this, I noticed the accelerator pedal did not move the fuel lever thru the entire travel. There was about 3/4" slack in the cable. Solved this with a wire rope clamp at the accelerator pedal end of the throttle cable. Now it has plenty of power and the transmission downshifts when required.
There seems to be no adjustment for the throttle cable. Some suggest the brackets may bend, but mine seem pretty sturdy. Don't know if the cables stretch, but taking up the slack made the truck much more drivable. Hope this helps anyone with the same symptoms.
