The one that goes from your intake manifold to your brake booster. Remove the clamp on the brake booster side of the hose, and put it into the can and start up the truck, it should just suck it all up.

NO, only about 1/3 of the bottle.

I'd put 1/3-1/2. The rest in the gas tank.

Get the engine hot and pull the seafoam in and shut it down immediately. Let it set 10 or so minutes and then start it and run it about 2000 rpm until the smoke show stops. Do it out in the clear hopefully so the wind is blowing toward the bad neighbor. The purpose is to burn off all the carbon deposits in the cylinders and valves.
