Warped Exhaust Valve

Anyone know what would cause the exhaust valve on a 3.0L warp like this?

That would be a burnt valve.

HEAT. The commentor above is correct, it is called a burnt valve.

As to a more specific cause, running rich, bad plugs, over-revving the engine for extended period, basically fuel burning up as it exits the cylinder past the exhaust valve (think blowtorch on the valve).
