vibration and wheel noise

My truck just recently started making some wheel noise( sounds like I have mudterrains), and my steering wheel has a vibration. I assumed it to be my wheel bearings. I jacked it up and sure enough I had a lot of play in my driver side wheel. I took the wheel of and saw that the 3 bolts that hold the hub were loose. I tightened them and there was no more play...but I still have the noise and vibration. I'm going to balance my wheels in shop class today, but I don't know if that's it because it makes the noise and vibration at low speed too. What could be the cause of this?
 
Maybe you have cupping or poor wear on your tires caused by spindle looseness.If you run your hand over your tires are they smooth or wavy.Try rotating your tires and see if it changes the noise.
 
Tires are wearing good and I just had it aligned, the noise just started last week
 
My steering wheel also starts shaking around 50mph. i unlocked my hubs and balanced my tires, but its still there. Any further advice, im going crazy:bawling:
