Vacuum lines ?

This may sound like a stupid question but how does one go about reading the diagram on the truck on vacuum lines. I must have a vacuum leak because I changed all o2 sensors and cleaned my MAF and still get codes 171 and 174. Any help would be greatly appreciated.
 
the diagram shows what need to be connected to what. the black lines are vacuum lines and the various comonents are labeled on the diagram.
 
Probably a plugged fuel filter, Could also be an intake gasket leak.
 
Don't mean to jack your thread here, but would someone explain the difference between codes 171,174 which if I'm correct is system too lean with P1151 Lack of Upstream Heated Oxygen Sensor Switch - Sensor Indicates Lean - Bank No. 2.
P1152 Lack of Upstream Heated Oxygen Sensor Switch - Sensor Indicates Rich - Bank No. 2.

I've been trying to figure out the later as these are constantly throwing my CEL. Even had the shop replace the intake gaskets, changed O2 in bank 1. Getting annoying
 
sounds like your bank 2 O2 sensor is getting lazy...might as well replace it as well since you already replaced the other side.
