Truck started while in gear!?!?

My wife turned the ignition yesterday without pushing in the clutch and the truck lurched foreward. The 12 years I've owned this truck, never had the truck started while in gear. Isn't there a safety switch that prevents this, and where is it located, and what may have caused this. Fortunately there wasn't another vehicle in front of her!

it is located on the clutch rod at the firewall
