trans issues please help!

I have an 86 ranger 2.9 2wd with a 5 speed and 170,000 km's. yeasterday i drove about a 70 km trip just to the next city and it semmed fine the entire drive until i actually made it to the city and got to stop and go traffic, i Have to be on the gas when im in gear no matter how fast im going if i let the engine brakes come on the tranny gets very loud and it jerks a bit, it doesnt seem to want to go into 4th gear and the others dont shift in as smoothly as normal, it seems like the clutch isnt ingaging or disengaging that well either, it got stuck in 3rd a couple times. I was really worried about getting home but i had a place to stay if i needed it so i tried not to worry about it and then a couple hours later i came out and it seemed okay almost just like it had overheated or something??? I made it all the way back and it did fine untill i got back home in stop and go traffic and it did the same thing, it has never done this before although the last time i made alonger drive it did start making more noise than usual while in gear.

this is my daily driver and i really dont want to have to try and through my project truck back together so i can drive it and fix this one so hopefully is just something small.

Check the fluid in the trans and check the clutch fluid. It is getting hot, but its because of 5th gear. Get it full of good oil and it will last long enough that you can rebuild it on your schedule.
