rpm's flaring?

does anyone know what would cause the RPM's to flare up? when i give my truck gas the RPM's start to flare even though my truck is not accelerating, it seems as if something (maybe the clutch) temporarily slips, and then begins to catch and begins to accelerate normally. it also feels like my truck has less power and cannot get up to very high RPM's now without slipping. if anyone has advice it would really help, thanks
 
auto - fluid level in the tranny can cause temporary surge, burnt 1-2 clutch pack

manual - glazed friction disk, diaphragm on pressure plate sticking, residual pressure in clutch cylinder due to line blockage or crimped
 
Perhaps if it is an auto the filter needs changing.. not enough fluid is getting thru it..
