RPM's Flairing?

does anyone know what would cause the RPM's to flare up? when i give my truck gas the RPM's start to flare even though my truck is not accelerating, it seems as if something (maybe the clutch) temporarily slips, and then begins to catch and begins to accelerate normally. it also feels like my truck has less power and cannot get up to very high RPM's now without slipping. if anyone has advice it would really help, thanks

You clutch is going. It is giving you warning now so that you are not left stranded some where. Better have it taken care of soon.
