Question about cold start problems

I have an 87 Ranger XLT Supercab 2.9L auto. I was having problems starting it when it was cold. If I gave it some gas and held it at a higher RPM it would eventually warm up and run fine. This has progressively gotten worse until now it won't stay running at all. Turn it over, it starts but then immediately dies. Give it some gas and it still dies just more violently.

I pulled the IAC and cleaned it but it didn't help. I tried starting it with the IAC removed, not unplugged, removed from the truck, it started right up but idled at an extremely high RPM. I thought that told me my IAC was bad so I bought a new one. Same problem.

The only code I am getting now is 18 - Loss of tachometer input/IDM circuit
failure / SPOUT circuit grounded.

My question is is this code 18 a symptom of the problem or a result of it? What should removing the IAC then starting and running tell me?
