I just swapped a 1354M into my 86 to get rid of the shortened slip yoke 1350 shaft I had been using. It installed without an issue and works well in all ranges.

It makes a noise, I could only describe it as a hiss while driving 30mph+ Fluid was full when installed, changed it, it was black but was clean with no metal fragments or metallic sheen. It hasn't seemed to get any worse in the 50-60 miles I've put on it so far. One thing I did notice is when the output shaft was turned back and forth there was a clunk inside the case, it took about 5* rotation to make this noise, I'm assuming that this may be some slop where the tang on the pump engages the case?

I guess what I'm asking is, does this seem like an issue to be overly concerned about?
 
All my cases make a light clunk when you turn the shaft back & forth quickly.

Not sure about the "hiss" noise though... Have you rechecked the fluid to see if it's discolored again?
Was the case you replaced an e-shift? (maybe the hiss you're hearing is simply vibration traveling up the shifter handle that is now present inside your cab)

I replaced a 1350M, mainly to get rid of the shortened driveshaft. I have not checked the fluid since I changed it but i'll check it tomorrow when I tackle my coolant leak.
