Pulled codes 1994 4.0 and got a 0136 lean bank #2.
Truck has 160K on it and O2 sensors look OEM.
I will check for vacuum hose leaks when it gets warmer out.
Might just replace the EGR as well.
I do get pinging and occasional smell like the truck is running rich.
Any suggestions would be great.

Just to add the idle is fine and the truck has plenty of get up and go.
Just have that annoying CEL that goes on and off while driving.

I got a 0176 and a 0189 code as well.
All things seem to point to the o2 sensor on the driver side(Pack 2)
 
Clean EGR valve before replacing it, could be leaking a bit of exhaust into intake.

O2 sensors are rated for change every 100k, so wouldn't hurt.
My '94 has over 300k and still using the original O2s

Also put a can of Seafoam in the gas tank, I do this once a year, seems to keep injectors clean and really reduces random misfires on the high miler.

Unhook battery's ground cable for 5 minutes, that resets computer

I have a can of seafoam in the garage that I never used.
Maybe I'll give that a try when it warms up a little here. Also watched a video of some guy pouring it on the brake vacuum line slowly while another guy held the gas petal down to keep it running.
 
Update,

I got under the truck to see how hard it was going to be to remove the Bank #2 O2 sensor and found it was lose. I decided to change it anyways.
Man,that plug at the back of the engine aside the bell housing was a biotch.
I reset the code via unpluging the paper clip while it was blinking codes.
Seems good so far,took it for highway and back road ride. :)

That pour it in the brake booster vacuum line can be dangerous.
People do that to try and clean off carbon build up in the cylinders.
Carbon build up causes pinging/knocking, so if you don't have that then don't do that, lol.
