I'm just looking for things that could cause it

Is this a new purchase with no history? Have you pulled any codes
Yes and no I bought it around 5 months ago it was this way when I bought it, it throws out 111 for the code it has a clean title

I would clean the throttle butterfly and the idle air control valve and it's passages. Each time you turn the truck off, a wisp of oily smoke will be inside the intake. After some years, this oily mist will build up on the throttle blade and the idle air valve and passages as a sticky brown build-up. This can clog things up and let it not get the proper amount of air at idle.
