Looking at an '09 - what to watch out for?

Between those two trucks and knowing nothing else about them, I'd be tempted towards the first one simply because the 5 speed has a better record of reliability than the auto although I think with regular maintenance, either will suit you well. Resale value when you're done with it will probably be better too... I'd pay more for a 5 speed truck with no options and high miles than an auto with worn out power everything.

Ranger automatics have never been great for reliability. BUT there are ways to fix that for under 100 bucks. Add an aftermarket cooler, put a drain plug in the pan, and in this one circumstance ignore the factory maintenance schedule. Change out the "pan" fluid every other oil change. (3.5 - 4 quarts). Drop the pan and change the filter every 4th oil change and your transmission will last forever if you don't beat on it.

I wanted a 5 speed manual when i bought my truck new but the manual cost more so i got the auto.

I would say rust issues...

Check core support and mounts... cab mounts... bed support rails.... spring hangers.... and have a good look at the bottom of the doors.
Rust on the bottoms of the doors and tailgate, lower rear cab corners.
