lean bank code help (176)

[94 4.0 5-spd 4wd ranger]

im currently getting a lean bank 2 code on my 94 4.0. i was getting the code just before i put a set of pace setter headers in and am still getting it with the headers and new o2 sensors. tried clearing the codes and it just came back within 10 miles. also went over most of the vacuum lines i could get to and they were good any input would be appreciated, oh also replaced the the fuel filter about 800 miles ago. wondering where i should go next, injector rebuild, fuel pump?

It is usually a vacuum or intake gasket leak.

I've seen some of the contacts inside the o2 sensor plugs push into the plug and cause lean codes from a bad connection as well.
