Hiccup @idle

I have had the truck for a year. It had a bad head/valve so the idle was always bad. I installed remaned heads. Truck runs good except for hiccups at idle. I cleaned the IACV but i don't think it is an idle issue. It idles around 725. No codes. I have heard that these Vulcans do have a crappy "idle" by nature. Thoughts?

Ps. When I start the truck it is in open loop. It warms up then goes to closed loop. However when i go to WOT it stays in closed loop. When i let of the pedal it goes to "deceleration open loop" i thought at WOT I should be in open loop?

Checked codes this morning and have a pending p2198. I also checked the AIT and ECT before startup and they were both 48.2F.

Yesterday i disconnected the MAF and drove for a mile or so. The truck did not stall and ran pretty good. It threw some MAF codes. I reconnected the MAF and cleared the codes.
