I've got a 1993 Ranger with a 4.0 5 sp man,and it is a 4x4. I was towing my boat last week up to phoenix during 100 degree weather. I was in 4 gear doing 2500 to 3000 rpms and going about 65 mph. North of Phx about two and half hours into the trip I lost all power and had to pull over. Tried the engine and it turned over but didn't start so I opened to hood and did the look thing, looks alright to me. Got back and it started up and I made it about 1000 feet and it died again. Cop pulls truck and boat off highway and suggest I clean the battery terminals and I did that. Madi another mile or so befor it died again. Went to make a call to get some help and had a bite to eat and the truck cooled down for about an hour. I was 20 plus miles to a hotel, it was memorial day so no help was coming that day. It started up an I end up driving back to Tucson(150 miles) that night when the temps out side were 25 degrees cooler and I only drove 55 on the way back.
The next day I did a tune up ,plugs&wires, cleaned the MAF, flushed the rad and put a new thermostat in and changed the fuel filter and the air. The next day I hook up the boat and head for a lake only 100 miles from here. Its late afternoon so it was only about 95 outside at the time. I made it about 60 miles and it died, on a two lane road with guardrail and no where to pull over. I got it to the side of the road and let it sit for 30 minutes and it fired right up and I made it back home. Right after it dies and I try to start some time it will start and run rough like its not getting fuel and when I pulled the plugs one had a frost on it if that helps.
I went and got a code reader and here are the codes I'm getting.

Where do I go from here?

Thats REALLY weird -
111 - Systems Check OK - You should never get any other codes with this one
10 - ??? I have no idea, how did you get a zero?
157 - Mass Air Flow signal is/was low or grounded - MAF
186 - Injector pulse width longer than expected or Mass Air Flow (MAF) lower than expected - Fuel control
542 - Fuel pump open, bad ground or always on - - Power / Fuel Pump Circuits
 
erase the memory and test it again 111 tels me pass hard faults 1 is the separator and the rest are memory start the KOEO test and when it starts giving you codes end the test that will erase the memory. Then run the engine untill it reaches operating temp and run the KOER test and write the codes as you receive them then get back with us.
 
It is best to drive it around at medium speeds and turn the heat and a/c on in all positions while your warming it up and test it while it is still up to temp.
 
After warming it up I did the KOER and got a 111. Cleared the codes and drove it to the store and back (25 miles) and tested everything again and both show 111 now.
