help! on 3rd throwout bearing in 4years...

The truck in question here is a 94 2.3 5 speed(of course). When I bought the truck 4yrs ago, I had to replace the transmission. So I went ahead and change the clutch,pressure plate,flywheel slave cylinder and pilot bearing. After about a 6. the throwout bearing started whining and about the 8mth Mark it exploded!. We checked to see if maybe the clutch fingers were bent but they weren't. So after about a year, same problem. Started whining and let go. I keep it well maintained and I don't drive the truck hard. Don't ride the clutch and I don't down-shift the tranny. Any of you fellas having this same problem? Or can you give me some advice?
 
never had any problems , are you using a brand name bearing? is the flywheel running true, also can you tell if the face of the bearing was contacting the plate correctly or is to damaged to tell, has the flywheel been resufaced a few times and is thin miss machined, also did you have a finger type pressure plate with a center ring( self adj.) or none finger type
 
It's a Durlast, it came with a lifetime warranty. After the 1st time it happened I pulled the clutch and pressure plate to check the run out on the flywheel. It was within spec and it didn't hardly show any signs of wear. Oh, and it's regular cast iron flywheel.
 
there are to different types of faces on the throw outs, a stamped one witch are junk and one that has machined cross hatch look to it, good one, I use the Sachs brand clutch kits, the stamped steel one works ok on center ring pressure plates but not on finger type
