Can't get rid of vibration

I had my tires rotated and after that I get a steering wheel vibration when I drive around 65mph. I took it in for my free balance, but it didn't change. I figured the tire shops free balance was worthless and took it to my dealership to have the oil changed and had them balance it. Afterward, I get on the highway and dammit if it still vibrates!

What is the problem? If there isn't a cheap solution, I'm just going to rotate the tires/rims back to the rear and leave them there.

My guesses;
out of round tire
slightly bent rim
I would think a good tire shop would catch those but you can check those for yourself. Jack the truck up and spin the tires by hand, look from the side and from the front. I also had a tire cause that problem when some belts broke and the tire was bulging badly in one area.
