So those two bolts are the ones I replace? Is there a certain spot where each washer and spacer thing goes? Also would it be a good idea to go ahead and replace the bushings too? The truck has 91k on it.

Yeah, those are the bolts, and sadly, by the looks of it the whole bolt will need to be replaced. On my 98 I got lucky and all it needed was the cams, and that was easy... Soak nuts in PB Blaster crank em off, remove alignment plates, install cams and tighten the bolts.... In your case the whole bolt will need to be pulled, and it could be tricky, so be patient.

Google Mevotech MK80065 and you'll find what you need... get two kits. And remember, Cams point up!

I've never had one get stuck or be a pain to remove yet.

You put em in so they look like this:

I'll wd-40 the he'll out of it and use an air rachet. So I replace the large shims with the new ones from the kit? And should I replace the worn rubber bushings?

Sorry for posting the wrong picture earlier.

The bushings are probably fine.
The bolts are a PITA to replace and you need to get it aligned directly after or your tires will really wear quickly. Do one bolt at a time and you might stay close to the original alignment, but you'll be tapping those bolts out with "something".

Watch the brake lines on the DS. I unclipped them for more give but didn't move them much. The new bolts go in just like what you take out, minus the factory plates.

Okay cool. This will be something I start on this weekend. Napa has the caster bolts and shims for 2 1/4 degrees. So I'll be tearing stuff down.

One last question, the new shims that are part of the kit; do they go on the outsides out the frame bracket, or in between with the bushings?

Outside...you will see the 'tabs' that they will ride in.

I don't have to label you now. You looked at it again and discovered your suspension was not the same :P
