Bad D-Shaft ate my Mazda 5Speed

Long story short is that a neglected driveshaft ended up costing me a rebuild of my 5 speed Mazda trans. I still have some issues I need some help with though.

1. the yoke that connects to the output shaft of the trans has been ruined so I need to replace it to avoid tearing up the new tailshaft bushing (and eventually killing my fresh transmission).

Can anyone point me to where I can purchase a replacement for this piece?

2. the axle end of the driveshaft joint (looks like a CV joint but not sure of proper name) is shot. I'm told the vibration from this joint is probably the original cause of death to my transmission. I'm told it cost more to rebuild the shaft than to have a new drive shaft, with traditional U-joints, built. I'm looking at $300+ for that.

Can anyone suggest a place to purchase a used or rebuilt driveshaft for less than having a new one made?

As this seems to be a poor design issue, is it a common issue that people have come up with work arounds or alternatives for?
