Here's a VERY good bunch of data and experiences from folks working through the 5R55E VB issues (clicky)

As for where to buy... I've been pleased with the lifetime guaranteed stuff from Wholesale Transmission Parts, but they do have some rocky history - Central Valve Bodies are also top-flite, call and ask for EXACTLY what you want.

Adding some sort of shift-correction kit (usually a pressure modification) and a new EPC valve would be bare minimum in my book.

Speaking in generalities though -- if you've been slipping for 3+ months (based on thread date) you may have other issues (worn-out friction material, heat glazing, etc). You may wanna include a good fluid flush/change to get the worst of the fried ATF outta your tranny shortly before you drop the pan to swap in a new/updated VB.
