why does my 4.0 shake and idle high on cold starts?

In the last few days when i start my 2000 ranger after its been sitting awhile, the motor seems to be shaking/vibrating pretty bad. it feels like im going down the highway and a tire is out balance....but obviously the thing is sitting still.

prior to this, i noticed it would idle really high on cold starts, then go away once it warmed up. now it just shakes.

anyone have an idea?

How's your fuel pressure on the rail at turn key, engine off and after start?

Sounds like i need to take it to the shop to check the fuel pressure. If it were bad sensors, would it throw a code?
