problem with 4th gear

ive got a 92 ranger 4cyl 5spd ive noticed just recently that when i shift into 4th gear it drives kinda rough it kinda feels like its in too high of a gear but i know its not i shift from 3rd to 4th at about 35 mph and from 4th to fith at about 55 and it does this all the way through 4th gear it just started doing this the other day on my way home from work all the other gears seem to be fine the truck has 220k on it and about 50 on the clutch ive been told that it may be the problem but i dont think it has anything to do with that i replaced the clutch with the same quality clutch that came stock and got over 170k on the stock one has anyone here ever had this problem and know what might be the culprit and what i need to do to get this fixed
 
uhhh... it is too high of a gear, you are upshifting to 4th at WAY too low a speed.

Even with 4.10 gears you shouldn't be upshifting into 4th below 45 or so
and 5th gear at closer to 60mph.

And I'll bet you don't have 4.10's
