I have had this problem for a while. I warm up the truck when it is cold. When it goes from open loop to closed loop it gets loaded up. I can rev it but jusrt off idle is runs rough until it cleans out. When I drive it cold a few miles then turn it off to go into someplace, when I came out it is loaded up the same way and will die at just above idle until it cleans out. I'm throwing a code for the O2 sensor, is this my only issue or do I need to check the fuel regulator?
 
if its running OK cold, then it probably isnt your fuel pressure regulator.

the PCM is sending an O2 code because the O2 is pegged on the rich side of the scale. since you know you have a rich problem, i doubt the O2 is lying. you need to figure out whats causing so much fuel to be dumped. a bad coolant temp sensor, MAP, throttle position sensor, intake air temp sensor are all possibilities and should be back-probed to see whats going on. a service manual for your truck will give you proper readings for all these sensors.
