Help with power loss on 94 ranger 4.0 2wd

Hello I’m having a loss of power when driving I’ll drive around fine then very so often the truck will lose power it seems to go away mostly after I get it to 3000 rpm or so but when this occurs and I press the gas either the engine won’t take the gas or it screams like I should be doing 120. I’ve checked for vacuum leaks none whatsoever. The truck starts every time no issues idles fine until it loses power then when I come to a stop light the rpm needle starts hunting for a comfortable spot and sounds like I’ve got a massive cam on it. I really like this truck I just can’t figure out what it’s issue is

Fist off, I can't imagine needing to rev to 3k let alone needing to drive above it?

If it's about automatic, I would check my transmission fluid level, smell and color.
 
Tough to diagnose with that info, assuming manual trans as posted in your info

Change spark plugs

Change fuel filter

Clean MAF sensor

Use a vacuum gauge to check for clogged exhaust
 
I will check exhaust today I had pulled it off from cat back to see if that would help and it didn’t
And yes it’s a manual trans has new plugs and wires as well as a newer fuel filter 2-3000 miles on that filter
