Help! Stalling out

I just moved back to PA a few weeks ago and, being winter and all, my truck has been stalling out. The colder it is outside, the worse it is. I have fixed an exhaust leak at the passenger side exhaust manifold, replaced the sparkplugs, and replaced filters... what should I look at? What else could be doing this? Anybody with any ideas whatsoever, please post it here.

When?

While driving, or only when stopped?

Manual or automatic?

Will it stall idling in Park or Neutral?

Is Cold Start idle high, above 1,100RPM?

What is warm idle RPM?

It will start up okay-ish. But then it will try to stall out when I shift into drive or reverse, when there's a load on the engine at low RPMs. Sometimes when I come to a stop sign as well.
 
I got a 3.0 manual, and when it's a cold day out (15 degrees or less) it will stall in neutral after I start it. But then once it'starts warm it will run like it does in warmer weather. My guess is if it gets better after a few miles of driving, that it just doesn't like to run when the engine is cold.
