Driveshaft / transmission output shaft sleeve [PICS]

My front U joint failed on my 2000 2wd manual ranger causing the transmission seal and sleeve to back out. Does anyone know where I can get a new sleeve (picture of mine attached)?

Also, how is it suppose to be oriented when installing? There is a key that protrudes on the sleeve but two slots in the transmission output shaft that it could fit in. Also, which end of the sleeve goes into the tranny first? Keyed or not>? Any other tips for installation?
