I think when i disconnected the battery it reset the ecu the truck also sounds a bit like a diesel when smashing the accelerator and idles rough but it has done that since i got it.
Based on the year in your profile, I'm assuming you have a 4.0 SOHC. I would look at your timing chain tensioners. You don't want to be letting that noise issue go. If the chain guides break, you are looking at a pretty big repair. It might also be partially related to your idle issue.
I was told they fixed the timing tensioner problem in the late 90s but it almost sound like a knock when you rev it but its only right as you smash the pedal and it doesn't make the noise under load.
For the SOHC, it was later, like 2003. In any case, the tensioners wear out over time and need changed. Some say 70,000 miles and some say 100,000 miles.

They can do that as well, depending on how bad the guide damage gets and how long someone lets the problem go.
And the symtoms for bad chain tensioners do not line up with what I hear.
That means that when they go bad, you replace them.
