compression leak (again!)

About a month ago had a compression leak at number 3 cylinder, replace head gasket, then had bad miss
Took to mechanic, retordown and found a piece of "keeper" stuck in intake valve. Removed all good for about two weeks. Now again pushing coolant out of system.
What should I be looking for? Bad gasket? Warped/cracked head? Poor workmanship?
I hate having to tear this down yet again! It has to be done. I'm already over $60 in parts and labor!
 
Sorry, forgot to put in, this is a 93 2.3L, 5spd. Almost 330k miles.
 
I would check for cracks I find them in between valves. I work at a machine shop so I have seen a couple.
