Clutch rod popping off of pedal linkage

I have a 1997 Ranger 2.3L with the 5 speed manual transmission. The clutch master cylinder rod is coming disconnected from the pin it attaches to on the pedal. This has happened three times in the last month. Each time I have replaced the plastic padal bushing/retainer clip with a new one and it will not stay in place. Not sure what to do next. Has anyone experienced this before? Any suggestions? Thanks!

I remember having trouble with it myself. IIRC the clip for it wasn't in the kit my stores carried, had to special order it.
 
Thanks. I have been able to find the clip and replace it (three times). The first clip lasted 10+ years, the second one lasted a month and the third one lasted less than 24 hours. Either I am doing something wrong on installation (I don’t think so, it is pretty straightforward) or there is some other problem with the pin on the clutch pedal or the clutch actuation rod eye.
