Sounds like a worn out clutch. Does the clutch slip at all?

In 4th gear at a low speed press the gas hard and see if the engine races without the truck speeding up like it should.

I've never heard a transmission chatter than sounded anything like a clutch does.

dosent slip at all i can go from 1st to 3rd and it wont rev up just grunts as it gets to speed.
the clutch is also very firm and grabs nice.

Have you checked your engine and tranny mounts? When I got my ranger, the tranny mount was broke, which made the clutch chatter during and for a short time after letting it out.
