Can anyone identify this sound?

I usually won't start hearing this sound until the motor has warmed up. It's changed in rhythm since last week.. and I wanna say for a day or two it was fairly quiet. Now it's sounding pretty loud. For a while it was a taptap, pause.... taptap, pause... it changes.
Just replaced exhaust manifold gaskets. All connections throughout exhaust are tight. I can hear the sound when I put a stethoscope up against exhaust pipe. Seems loudest directly on cylinder head, however doesn't seem louder when I open oil fill cap?
I just replaced the timing chain tensioners as well, the screw in hydraulic ones. I never heard the death rattle, except for a couple times when I was on an incline and it would go away, and it wasn't that loud. More so preventative. Haven't heard any rattling since the new tensioners.

I really am frustrated at this point. I haven't taken off the valve covers yet, as I understand it can be lifters. I will probably be trying marvel mystery oil soon in case it's a stuck lifter. I've never replaced a lifter before. Truck is a 2005 Ranger 4x4 with 4.0 SOHC and 5r55e.
I have the entire service history of the truck and it's gotten regular oil changes. Only 120,000 miles. I bought it from a county mosquito exterminator. 5r55e blew on me 2 weeks after I bought it, I believe because someone had the vent hose plugged. I got a reman 5r55e, now dealing with this. Thanks in advance for any help!

 
I listened three times, It almost sounds like a valve is bad, but, you have that odd repeating pattern which makes me think it might be the timing . I never had a 2005 4.0 apart (though I owned a 2005 Monty) but, I think they have timing gears at the back of the engine and once the back goes bad, the engine is basically totaled, since it has to be removed to fix it.

Sounds like you need a valve adjustment...to bad you can't do that on a 4.0L RIGHT!!!

I would pop the valve covers and inspect the rockers and pushrods. If your lucky, they are worn beyond service, if you are even luckier, you can correct it with new replacement parts.

If you are the LUCKIEST, your rockers and pushrods are worn beyond service and you decide to replace them with an improved system:

• Hardened rockers from Delta Cam of Tacoma Washington.
• a custom set of adjustable pushrods from Smith Brother's.

Then, in 30k miles, when the you need to adjust your valve train (lifter preload) you are able to make the simple adjustment, instead of hearing the noisy valve train again and not being able to correct it with a simple adjustment.

@gaz the 4.0L SOHC has no push rods. It has an overhead cam.
