It is likely one that slipped thru the cracks
It didn't slip through the cracks. It had serious safety issues and was straight up ignored. He was assured the truck was in good working order and had passed all it's inspections and safety checks. They never bothered to even look. Finding problems would mean costing them money, and they put this guy's (and others) life on the line because they're cheap. By buying a truck and having a quality inspection and assurance deal with it I'd sure as hell expect the thing to at least stop.
It's no wonder they're doing as poorly as they are. Take 'em to the cleaners.
