1993, 3.0 bad idle surges help

I have a 1993 ranger. Its all stock. It starts fine, and drives fine. But when i'm sitting still at a red light or when i start it up it just doesn't seem right, it shakes and sends a little vibration through everything. it will idle and stay running at 1000 rpms but just shakes like it is surging. does anyone no what it is or have an idea. I'm stumped. it also falls on its face on take off, if you don't wait a couple seconds. thats no big deal, but i want it to idle right. any thoughts would be great. Thanks

I would try pulling the codes first to see if anything show up. Maybe check for vacuum leaks also.
