The low side port will be between the evaporator and the compressor. The high side port will be between the compressor and the condenser.
As an aside, if you cannot find those, perhaps you should leave servicing to someone with a bit more experience. There is danger of high pressure, freezing fluids, and hot stuff if you do the wrong thing.
R12 is becoming harder to find because it is out of production in USA. It may still be manufactured in China, but so...
If you are converting to R134a, there are several steps to follow. Flushing, removing as much oil as possible, replacing the accumulator and OT, adding new oil, assembling, evacuation and final charge are the basic steps.
FWIW, I think your system had a compressor that wore the teflon off the pistons, and created a goo called 'black death'. Such condition requires replacement of the condenser, OT, accumulator and a good flush of the evaporator, along with replacement of the worn out compressor. The goo cannot be removed successfully, and will continue to migrate around blocking refrigerant flow as it sees fit.
