1991 Ford Ranger running problems

Our Ranger is hard starting and seems to be idling rough, when its put into gear it will idle really rough barely run and then stall. We are running out of ideas here with this thing, even checked the timing chain (thanks to a mechanic who figured thats what it was, alot of work for nothing btw). So if anybody has any ideas here please let us know,

What has/has not been repaired or checked on it?

Hi 86Ford thanks for your reply the engine is a 4 litre, we have replaced every sensor under the hood, also changed the DYIS box just because, checked for vacum leaks and haven't found anything. The truck has alot of k's on it 428k's the engine is in the high 200's somewhere, it passed the aircare test last year with flying colours which usually means things are working as they should. The fuel pump is two years old, and we changed the filter. ARRRGGGGG
