Whobbly rotor

My right front rotor keeps coming loose, I know this because it starts to make a thumping noise at higher speeds, the other reason i know this is because every time I tighten the nuts or whatever they are, they always seem to come loose, I wish it was a crown nut that way I could use a pin to keep it there.

Maybe I'm missing something?

I'm sloppy with grease, haha, don't worry, after this photo I cleaned it up.
 
I'm assuming you know how to set up wheel bearings here...

The outer lock needs to be torqued to 200-250 ft lbs.
 
Hard to tell with the grease on it, but am I not seeing the locking washer in between the two nuts?
